Travel Assistance Program (TAP)

Medical Assistance: pink forms available from your doctor’s office for medical appointments requiring travel by ferry (in some instances by plane). They must be completed by phone with an automated service 24 hours per day and will cover all return ferry expenses for the patient and a passenger vehicle (and escort if indicated by your doctor). They are also available at hospitals for patients that arrived by emergency transport to return home.
